Horrible service

I called last week friday and I requested payment forms to do my surgical ********. Today is monday received nothing and I got to keep calling to request these. My appointment tomorrow at 9am and I am stressed out has I would not like to arrive at the clinic for nothing and be told to come another day. This is not the service you give a woman having to go through a ********. Please !
