Why sitting with the forms

I resigned on the 30th of July 2015. I was called by Yiva to sign Providend Fund Forms, which I did on the 7th of August. I sent an email to Alexander Forbes to check why am I still not getting my money, they told me no forms were received. That was the 12th of October. Why would you sit with the forms for this long?
