Bank account debited for a loan that was never taken

I am amazed that Loanspotter South Africa claims that I opened an account with them. You refer to this account as the Convenient Service Package with an activation fee of R499.00. I vehemently deny ever agreeing to this contract. Can you please provide me with documentation (either written or audio) that proves that I agreed to take up this package. I will continue reversing the instalment of R99 on this account until you cancel it. Interestingly this amount has risen to R1 306 as at 23 December 2017.
