Lightbody refuse to Return Deposit

I submitted a form in Mid April stating that I will be vacating their Room at 30 May 2014. When I went to request for my Deposit on the 1st of June 2014 they refused to return my deposit closing that I had not submit the form stating my intention to vacate their room.
