Lifeline debt solution ***ards

Hi guys im struggling with lifeline debt solution. I was placed under debt review by them in 2012 but that same year i canceled it. Then i got a letter 17.4 from them saying i voluntary canceled it. But the debt review is still appearing next to my name on the itc. Can someone pls help me coz their contact details doesn't even work anymore. Please i need help urgently
