Licensing department lost licence and does not order new one

My daughter had app**** for her new drivers licence in February 2019 , she has been visiting the licencing department every month since May 2019 and sadly the department has lost her Licence, even though shes been falsely assured countless times that a new license will be ordered. The supervisor was of no help neither were the employees, she's sent back each time she goes to the department having to take a photo and fingerprints again under the pretense that they will order a new licence which seems to be untrue as the department still does not have her licence on the 22 October 2019.
