Why Debit Me when I am not your client?

I got a debit off my account(R85) from Liberty life ref Liblife05apr5935. I do not recall having/taking any policies this year with you. My ID number is ********** 140186 and phone number ********** 667.
Kindly advise on details regarding what may have transpired for you to debit my account.
