UNFAIR COVER BY COMPANIES

Hi my name is Winslow Clouts can you please engage with my Employer to send me a Disability Benefit Statement because the don't want to send me one from Liberty. I was in contact with numerous time with Goldfields liberty branch and the call center agent said I am covered under Capital Alliance. But I should not fill or sign uncompleted forms or forms without a Disability Benefit Statement with a Liberty Letter Head on it, which was confirmed if was sended to the Company. Or can you please send it to me.
