Terrible Service from Zandile Ndlovu ( Customer Service)

I logged a service request to withdraw from my provident fund and I received a response from Zandile Ndlovu a week later. She asked that I complete a withdrawal form but did not attach it. I responded to the mail asking for the form with no luck. Is this the after sales service that Liberty gives clients?
