STILL NO PAYOUT!!

Still waiting on my Pension payout, now everyone is quiet, after having me re-complete the forms as instructed by the company advisor. I left my employer end of September. Filled in all the forms for my pension to be paid out in cash and waited. Got an sms saying claim cannot be processed due to section E1 of the withdrawal form that is not completed. Filled it again, I DO NOT WANT TO RE-INVEST! I want my money paid out in cash!! Please assist asap.
