retirement annuity cash out

<p>I have a retirement annuinty policy with Liberty and when i wanted to cancel it they sent me several forms to fill in, I tried calling to ask what form should be filled out as the others wasnt my duty to fill out. I call and call and several people said they will call back. Up till today I still dont know what to fill in. I wont use Liberty Life again because im disgusted at your poor service, you dont have a problem taking my debits but when I want to cancel my own policy nobody can assist me in a proper manner.</p>
