Provident Fund Withdrawl

I recently left the company I worked for and requested that my Liberty Provident fund be paid out to me and from the 14th of November up and until now I have heard nothing back from the most pathetic and useless brokerage company I have ever dealt with!! I have all my details of my fund sent from Prism Employee benefits where I got my membership number and details about the fund. I then contacted these useless people on the 14th of November to withdraw from the fund and to this day I have heard nothing. I receive documents to fill out and I send it back on the same day and they still do not respond or give me any feedback as to what is going on with the with drawl. I have sent an email directly to Liberty Life and the only response I got there was me copied into an email sent to this broker?? So PLEASE can someone get in contact with this brokerage and sort out my fund as I need to know what is happening to my fund and when it will be paid out to me????

I do not even have a name of a person working there as they are clearly too *********** to add one onto an email signature.

They ask for my contact number and when I gave it to them I still hear nothing.
