Not wanting to assist me with uif 19 Form to apply for UIF

I used to work for this company Liberty for more than 4 years and last year I got dismissed because I had lost my mother and they didn't even give me time to mourn my mother instead I got dismissed so now I' have been asking them to please help me with uif19 form to apply for my UIF and they all ignoring me, my previous manager Natasha arendse and my previous payroll Dikese nqae even tried reaching out to they senior manager Brenda Ntuli but they all ignoring me and I need to apply for UIF because I have a 4 year old depending on me
