NOT PAYING OUT MY PENSION.

I left my employer end of September. Filled in all the forms for my pension to be paid out in cash and waited. Friday I get an sms saying claim cannot be processed due to section E1 of the withdrawal form that is not completed. I DO NOT WANT TO RE-INVEST! I want my money paid out in cash! Now the broker is ignoring my calls, and the employer. Please assist asap, or should I contact a lawyer?
