Life Policy Issues

I have until recently never had an issue with Liberty Life. A month ago I noticed that a policy was added to my portfolio, upon querying this with Liberty, I was told that I initiated the policy and that there was nothing that they could do. The policy in question belongs to a Mr Sean Mark Pillay and when I explained that I was not that person they said that they would look into the matter. This has become an issue for me and my family as I am in the process of finalizing my Will and this policy is of concern to my family. I have made my intentions very clear to the several call center agents but it seems as thought they don't give a damn. All I ask for is that this policy be removed from my profile and updated documents sent to me.
