liberty life failure to pay out provident fund

my name is omar matthews. due to a hart attack& subsequent by-pass surgery in 2013, in september 2014 after numerous meetings with my company h.r, it was decided and agreed upon that i could no longer fulfil my responsibilities in my place of employment. since i was a liberty life provident fund member, i submitted a claim for disability, due to the points above. in june 2015, after having to supply liberty life with endless documentation and proof of my inability to fulfil my work duties, liberty life rejected my claim, basically not being satisfied with the proof submitted to them.i accepted the outcome and then went ahead to put in a claim to withdraw my normal benefits.this was done in june 2015. since the claim was submitted, they have requested additional documents on an ongoing basis. and to date, 10/09/15, have yet to receive my pay-out. i can confirm that they do have my valid banking details and my tax affairs are in order.i mail them frequently but still cannot get any joy from them. let me stress that it is now 1 year, since the initial disability claim was submitted, and 3 months since the normal claim for my funds has been submitted!! i am now planning to go legal
