Lapsed/Cancelled Policy Number ***
Dear Sir/Madam,
I, the undersigned hereby write to your office regarding the above matter. That is between myself and the Liberty Group. They are refusing to take responsibility hence I am approaching you. The policy in question was taken by me and it covered both me and my deceased wife. Inception date: 2009/01/01 and I paid its premiums from my mortgage account number: *** on the 15th of every month, on date. And that is done by stop order by my employer, Department of Correctional Services. In June, 2016 I got a letter dated 24 June 2016 instructing me to fill in and send back a new debit order form that accompanied that form. I will attach the letter if possible. Due date to respond was 2016 August 31. I filled and subsequently faxed a new debit order form on 2016 August 02 and asked Liberty to debit my cheque account number: ***6 from 2016/08/15 and onwards. I was taken aback when i was told that my policy was lapsed/cancelled. Reason being I failed to honor my debit orders for December 2016 and January 2017. (again, I will attach if possible) What boggles the mind is that lapsed/cancelled letter was dated 29/12/2016 and question is, if my policy was cancelled in December, what was Liberty doing in my cheque account in January 2017? Upon enquiring at Liberty Group Eastgate, it emerged that their system was updated to debit my cheque account on the 2nd of every month instead of the 15th August 2016 and onwards. I have a new debit order form which has all my correct details as was demanded from me by the Liberty Group. I furnished Liberty Group with everything that they asked of me and I believe I know that the information on the new debit order form and the one on my Bank Statements are correct and accurate. I chose 15 August 2016 and onwards because it has been and still is my payday.
