Disappointing service from financial advisor

Such a disappointing experience with Liberty. I urgently needed cash 2 months ago, and contacted my Financial Advisor, who promised to get back to me with a form I needed to fill. However she never got back to me. I ended up finding the form on my own, now that I have filled it, I can't get hold of the financial advisor.

She has not been responding to my emails, my calls never go through on any of the Liberty numbers. Is there no other way to contact the company when the person assigned to assist me decides not to do so????
