cancellation of my retirement annuity

My name is mzwandile Zulu, in March I requested to my broker to cancel my R.A. on March 25 they sent me a form for a cancellation n refund of my contribution. I waited for about a month for a feedback but they never bother n today I phones their CustomerCare to Enquiry about the progress but they told me they sent a wrong form and I need to fill up another form and she said she was going to put me through to another relevant division for that is responsible for cancellations. I waited until they hung up on me I phoned again until I realized that they are not willing to help me. Im willing to withdraw everything I have at liberty because they are showing incompitancy and that they can't be trusted. I'm no longer willing to invest with them.
