********** account under my name

In 2013 there was a ********** account that was opened at lewis store under my name. I only found out in May 2015 that I'm on credit bureau because of Lewis, I did the follow up. I called the head office and they confirmed that there is an account under my name. Ever since May till today never get help from Lewis, I try to open a case on this but I found out that Lewis already open a case on this matter but the case was closed because there was no enough information. I have been calling, sending emails requesting the contract of this account but never get anything from them, and the police claim that they can not continue with case. I have tried all means to get this information with no luck, now I decided to report this to Hellow Peter to assist me with this because I don't have any other option now. All I want is those document so that I can continue with the case. Funny enough this future was delivered to certain address but today they don't wanna mention anything about that house. I hope and trust you will find my complaint and assists me.
