Club Magazine Added to account without my consent

My mom has a Lewis account that I am paying as she has become ill over the past year. When we opened the account initially, we advised the sales person Colleen that we do not want to join the club which is R 25 per month. The 1st three months of the account being opened, there was no club magazine. Last year June, I noticed on the statement that club is now on the account. I phoned the branch to cancel and it was never done. The post office were on strike and we did therefore not receive any statements. When I looked at the statement in January, I noticed my mom is still being charged for the club magazine. I want to know why it was added and who gave consent for it to be added. I want all the club payments on this account reversed as I pay way more than the installment to settle the account as soon as I can
