I AM STILL WAITING FOR MY FORM

My husband was admitted in an emergency at Lesedi clinic on the 2/7/2015, and it happened that a cardiologist had to assess him to check if he didn't have any heart failures? he was then discharged the next day, however the medical aid then requested a PMA form to be filled by the Dr so they could process the claim and pay both the hospital and the Dr, however to this day, she hasn't filled the form that doesn't even take minutes of her time and she will also get paid by the medical aid, we have been calling almost every two days and we also went to the hospital to ask the Dr to sign the form, however she kept on advising that she will do it and have it sent, we even asked the medical aid to contact her office and she advised that they should expect the form on the 21/07/2015, when I called today (Medical aid ) they told me she never sent the form??????<br> This is poor service coming from the hospital, how do you recruit such lazy Dr's? I am also waiting for feedback for the complaint I logged through the clinix website<br> <br> <br> <br>
