LANCET ERROR STATEMENTS WITH INCORRECT DETAILS

I AM STILL WAITING FOR MY STATEMENT FROM YOU GUYS , THIS STATEMENT NEEDS TO BE FIXED , MY INITIALS IS NOT " J WILLIAMS " IT IS " SD WILLIAMS " HOW AM I SUPPOSED TO CLAIM BACK IF THESE DETAILS ARE INCORRECT , FIX THIS MESS IMMEDIATELY AND SEND ME MY STATEMENT VIA EMAIL ASAP
YOU GUYS ARE VERY UNPROFESSIONAL WHEN I HAVE BEEN ASKING FOR THIS ERROR TO BE FIXED FOR WEEKS NOW .. FAILURE TO DO SO WILL BE REPORTED.
