Lab Request Forms not submitted

I attended the lab on 08/01/2015, I started receiving messages 2 months later stating that my account is unpaid and I am liable for a certain amount. I am on medical aid so I contacted my med aid company and was told that they did not receive a lab request form. Following numerous calls to Lancet requesting the forms, I called again on 27 May and was finally given a clear answer as to why the med aid may not have paid the account. I was told the amount to be paid is for an added test which \someone"requested telephonically(Im assuming the doctor) so there is NO lab request form. My issue here is that I would think it is the responsibility of Lancet to ask the doctor to send through a lab request form for the added test so that it can be processed by medical aid. Now I must pay an account eventhough I am on medical aid???? And is that how the lab operates"
