Subject: Urgent: Correction of ICD-10 Code on Invoice and Resubmission to Discovery

Dear [Practice/Billing Office Name],

I am writing to follow up on an urgent matter regarding my recent urine test results. The incorrect ICD-10 codes were used on the invoice submitted to Discovery, which is causing a delay in payment processing.

The correct ICD-10 code should be N31.1 (Neuromuscular dysfunction of bladder, nonobstructive), but the invoice currently reflects incorrect codes. This error has been flagged, and we were advised by Dr. Zaheeda Adam's secretary to contact you directly regarding this issue. However, we have not received any response since our previous communication on 30 July 2025.

Please correct the ICD-10 code on the invoice and resend the updated invoice to Discovery as soon as possible to avoid further delays.

Kindly confirm receipt of this message and provide an estimated timeline for the correction.
