Dirty/Soiled Cotton Wool

I was at the Morningside Clinic Lancet just before 6 for a blood test.The lady who was taking my blood took off one of her gloves before inserting the needle in my arm. After extracting blood from my arm she used a brown looking cotton wool on my arm before she app**** the band aid. I had to point out that the cotton wool looked dirty because she didn't even notice. She said she took it from the clean cotton wool tub. I'm shocked that proper procedures and more care is not practised at an institution that handles something as sensitive as blood. How risky is such an incident? Please clarify this for me.
