Bad Service at LA Health

I am asking for help about my medical aid. I have changed my medical aid in December 2018 to LA Health (Discovery), I have submitted the forms on the 14th December 2018. I was told I will receive a letter that works like temporary card by January 2019. Then a actual medical aid will follow after six weeks. On the 10th of January I phoned them to ask about the letter and I was told I didn't fill up the forms with banking details of which they didn't send them to me . On the 11th of January 2019 I went to fill up the forms again and was told everything is fine. I'm still waiting for the temporary card even today.
