Kulula credit card cancellation - worst service ever!

I have been struggling to close my Kulula credit card for months now. The worst customer experience ever with the call centre! You should rather cancel this service if you can't assist clients. They keep on promising to send a form that I need to complete, but I never receive it. Please can someone that can actually assist contact me to sort this out.
