Bad service - Refund

I purchased a ticket for my uncle to fly from Durban - Jhb. Before he could fly he landed up in hospital. Since Nov 2018 I have been trying to get a refund. Every time they require a different form from the hospital. They are giving me the run around and are refusing to refund the ticket
