Injury at the KFC store - No response

I was injured at the KFC in Pta, the staff were quite helpful initially by calling an ambulance for me, when the ambulance took a while to arrive and I was bleeding profusely they then got me a taxi to take me to the Hospital and never bothered to check on me. After the hospital incident and a bill of close to R2000 I then went back to the KFC to let them know that my bill resulting from the injury was close to R2000. They offered me a streetwise 5 and expected that that would be the end. PATHETIC, I have decided to launch an injury claim but do not seem to get any response as to were I can get the claim forms. KFC NEVER RESPOND, I will make them respond.....
