Still waiting for UI19 for 2010

I was working at Absa bank through Kelly and my assignment ended on the 31st March 2015, and I went to apply for Uif and all the documents was needed to make a process my application and I got them from Kelly. After submitting those documents I was told that they still need UI19 from 2010 of which I was working at standard bank through Kelly also at that time. So I have sent Zarina email and request for it but no luck<br> I have laid a complaint against against labour DEPARTMENT for not paying me 2 weeks ago but still told me the same 2010 UI19 story so I have sent another email to Zarina to please send me that for but she just didn't respond. I need somebody to help me please, I am unemployed, I don't have any money to even call Kelly to request that form because they don't respond to my emails. I have been staying home for 5 months already with no salary, could someone please have a heart and help me with that form cos it appears on labours system that's why I can't get paid. Please please I am begging now for my money that has been deducted while I was working so that it can help me in time like this
