still waiting

I have contacted kelly head office and kelly vanderbijlpark on a couple of occasions to collect my ui19 form that was suppose to have been handeled in 2011 and so far one rude lady from vanderbijlpark branch had the cheek to phone and give me hassels causes I put a complaint here, but she hasn't resolved my problem and doesn't seem like she has any interest in doing so! How diffucult is it to end my employment from 2011 at the labour dep!! Just complete my ui19 for goodness sake!! And don't pleases don't respond with your copy past we will investigate message, sort it out its bee 2 months!! Seem like I have to lock calls at customers complaint and carte blance just for a responce!! I don't want a phone call from that vanderbijlpark women again! Rude and unprofesional
