I never received UI19 form

good day I was on a learnership programme from kelly group 2014 february to 2015 january and the learneship end i was employed by quest there after and now I am not working and i went to department of labour last mouth on 22 the told me that i should get the UI19 from kelly group and quest. I went to quest at braamfontein and i request the document for both kelly and group cause kelly move to quest, I was assisted by breda motshabi matlaopane she under quest and i did get form for quest and for kelly group she told me that kelly join them 2015 and she need to request it and on the 14 september 2017 i received email from ********** saying that there are no records for attached request sent from ********** email detail from theresa fortein. and i went to department of labor again they told that if kelly say there are no record the need to give me the official letter so that they would assist me with the claim of UIF. then i went to quest and i told brenda matlopane that this is situation i am having and she said from they side they dont have information from 2014 and she said she will send it to IT people to pull up my request it and i must wait for feedback she will inform me and she never get back to me. then i email ********** to tell them that i went to labour the need officail letter if the is no record it was the 21 of september 2017 and the never respond to my email and i also did email them again on 27 to ask what is happening and the was no respond. I am very sad and angry cause now I cant claim my UIF because of kellygroup it is bad service for me not getting what i real need kindly assist me it been long. i have past the supporting document. regards

I am zenzele brian Shabalala my ID no: ********** 660087 I was on a learnership programme 2014 from February to 2015 January it was letsema learnship programe and when i went to department of labor they told me that they need that UI19 form from you and if there no record i should get an official letter from you i try to enquirer this with breda from quest at braamfontein and she told me that the is technical issue of getting information from 2014 cause kelly join them 2015. Kindly please assist me i am desperate and i real need to do UIF claim please
