The worst health insurance

I visited 2 doctors twice each but according to them I owe them. I called them twice for preauthorisation after i was informed by them that i have to call them. But now they are claiming that i visited a doctor 5 and i owe them, i contacted them each time I visited them. They are trying to **** me. This is the worst health insurance ever. I have done momentum and discovery but this one is the worst. Even their benefits suck.
