Declining Authorization after approving it.

My Mom fainted Wednesday night (23 August 2023) and was rushed to the hospital and the doctor said she need to be admitted we called for authorization, and it was approved but 6 days later (29 August 2023) I get a call saying Kaelo decline the authorization after the have already approved it and now they deny ever approving it. and Kaelo have been getting calls to authorize the tests she was undergoing, and they were approved so now they are saying it's not one of the diseases in the cover so they won't pay, firstly why would they decline the authorization after approving it and secondly why after so many days.
