Wrong completion of ui19

I was retrenched on 04.09.15. When i went to dept of labour i was given forms to submit to hr. I went to the Pta west branch where i was working and the B.M. emailed the forms to Hr. I waited for almost 4 weeks cz i ws tld that she's on leave.When she returned them the dates were wrong cz i didnt start working at Russells in 1989, i started in 2006. The consultant at labour sent me back to Russells. I showed the B.M. the mistake and she emailed to Hr again. When she sent the ui19 it was wrong again and the consultant wrote on it just to give an example of how they want it to be completed. I sent her an email telling her how to complete it and tld her that the consultant said if she doesnt understand she mst cl them, she ignored my email til today. I went to the branch again on 01.02.16 and the admin manager emailed again and when she responded she said she helped over 1000 ppl and she neva had any problems y is mine a problem. My claim was declined on 28.01.15. So i tld the A.M. to ask her if she wants me to forfeit my money or what.Im tired of going up n down for mahala cz she doesnt eve? care that im spending transport money and everytime i go there my dates are wrong.
