not handling provident and leave pay, sending UIF papers

I finished at Russells almost 5 months ago and have not received my leave pay, provident fund payout and my UIF papers, my colleague and I are kept on being told the claim forms are sent to Alexander Forbes and our payout would be coming soon but upon liaising with Alexander Forbes emails were sent to me that head office never sent the claim forms (which happend 3 times) and when we enquire about the lies they seem to get frustrated with us, were as we are the ones who are frustrated because we walk up and down and phone to no avail, and the sad part is that we are 3 that have this problem that nobody wants to help us with. would someone please assist us.
