Remove my account on ITC

I left Iwyze insurance October 2017 but my account still appears on the ITC report, Please remove my name because I don’t have an account with you. I have been trying to apply for home loan but I get declined because it seems like I have too many accounts which I don’t, Auto & General also has the account still opened on ITC. Sort this out please.
