Life policy doesn't exist - debited for 12 months with proof

I app**** for a life policy with old mutual, did the whole process including the debit mandate. For almost a year now I have been debited for a life policy that doesn't exist. I went to old mutual branch on the 20th June, Logged a query and I was told I will get feedback in 10 working days and nothing thus far. 10 days is long past they debited my account again for a policy that doesn't exist.

I wonder what would have happened had I passed away ( God forbid) and my family went to claim and be told I had no policy with old mutual while old mutual is debating me for a life policy for the past 12 months.
