incorrect recording of information: life cover contract

Old mutual has listed incorrect information on our life cover policy that could affect our cover, we answered questions correctly and stated that we have vaccinated for Covid-19 but on their contract , old mutual omitted to state that fact and said we were not vaccinated,

We would like old mutual to rectify the information as it could affect our cover and claim at the later stage, and I am not happy with that false information it is prudent that the information provided by us is verified and correctly recorded.
