False Debt review application

They put a debt review application/grant link on my ID. I AM NOT ON DEBT REVIEW!!!!! Now my credit score is done for. I cannot do anything, when I call them, they say that NCR placed my name under the review and not them. I have been trying my absolute best with NCR but its been months and I still am not getting it off my name. This is affecting me so badly :( I honestly dont know what to do anymore!!!
