Unfair labour practices

I'm no longer working for Impendulo labour solutions as a safety rep but was told I resigned of which I never did however when I requested my UI19 FORM I was told by the CEO Peter adonis to go look for it by the ccma ...I went to the ccma only to be told that it is the duty of the company to issue you with a completed UI19 form ....it seems like Peter don't want the staff to get their uif monies ....did he even pay it to the department.....our contract also states that we entitled to leave or or leave payout when you no longer work ....I'm told that I don't qualify for it....please assist /guide where possible
