Unknown policy

There is a policy that was taken up under my name on my behalf, by whom, I do not know. This policy started in 2018 apparently. I do not recall having such a policy but due to the fact that it is now in arrears, it is causing me to have problems during credit checks for job applications, rental applications, and even for credit applications. I have been calling IDM since last year and to date, the policy is still showing as though I am in arrears after numerous calls and being promised that after 3 months, it would be removed on my credit report, but has not been removed. I recently called IDM on the 17th and the matter was escalated but there has been no communication ever since. Can I please be assisted in removing this policy in arrears from my credit report or profile, as it is really affecting me negatively.
