2 premium increases 2 months apart

I am not happy that I have had a second premium increase in a space of two months and without notification. My annual review was in June and I had an increase because of a claim in the prior yr. In August my premium went up again and when I called to enquire why this was so I was told this was a once off adjustment related to the claim I made last year. I requested a copy of the policy as I had not been informed of the adjustment and to this day it has still not been emailed to me. The September debit order went through and it's the same amount as the Aug premium yet that was indicated to be a once off adjustment. Pls explain!
