Homechoice ***** account under my name

The is ********** account under my name and I never opened any account with homechoice because I'm not working, I have received calls from homechoice agents and I have told them several times that I never opened any account with homechoice. I was advised to write an affidavit and sent it through via email, which I have done but I never got any response or any confirmation that they have received my email. The only thing I'm requesting is that the account should be removed under my name because definitely it's affecting my credit score, how can I open an account while I'm no longer working?
