Homechoice *****

I was contacted by somebody at Homechoice regarding ********** activity on my account. I have never heard about Homechoice nor have I ever bought anything from them. I contacted them and it turns out somebody opened an account with my ID number and under my name and bought goods to the value of almost R4000. It has been a week now that I call homechoice every day in order to get this sorted out. Every time they promise me that they will send me a form that I need to complete in order to get their ***** department to look into this matter. Apparently without this form they won't start the investigation, but they don't send me the form.
