********** APPLICATION ON MY NAME

My Name is Maria Mokoena, the last time i dealt with Homechoice was in 2006. This Morning i received an email from ABSA that they have received a request from Homechoice to disclose my bank statement, and if i did not do so i must call them. On calling them i was told that they have indeed received that request, i was adviced to call Homechoice to find out because i told them i did not apply for anything with them. I called Homechoice and was assisted by Liezel she confirmed that there is an application on my name done on the 7th Nov in the morning, she refused to tell me who is the employee and what what odered. She gave the reference ********** 55 and told me fax the letter and my ID to home choice again, how can i fax my details to people who want to de***** me, i want Homechoice to send me records of this call and disclose the name of the employee who did an application on my name, and the numberthat was used.
