********** account and ********** debit orders

Good day, I see on my credit profile that there is a Homechoice account linked to me in excess of R6k. I've never app**** for an account. My bank account has just been debited for R620, I've never approved this debit order. I emailed your customer service and was told that i need to supply an affidavit to say i never app**** for account. How about you supply me with you evidence that i did apply for an account. Please send me the application form with my signature. I need this resolved ASAP as i do not have these extra funds to line some ********** pockets, i need MY money to feed MY family!
