Demanding payment for unknown order

I always receive calls from Homechoice demanding the payment of some mentioned items that i was not even ordered them. when i explain that i did even ordered and received those mentioned items they request me to make and send affidavit and copy of ID to state that i did not ordered anything from Home choice. I sent it several times but still no progress made. as i explain my name is listed on credit bureau because of that matter please clean my credit record. I tried to request a copy of delivery note, any proof of good received, copy of ordering form or if is telephone order any recording but they failed to produce that. My ID ********** 579087 My contact no. 07 ********** 2. Am not getting any credit or job because of the Home Choice could you kindly attend this matter urgently.
