Account incorrectly opened with my ID number

I recently found out that there is an account opened under my ID number at Homechoice. I have never opened an account with Homechoice or purchased anything there. I called the store and gave them my ID number and they informed me that an account is open under my ID but the names and address of the account holder do not correspond to mine. I have been asked to send a certified copy of my ID and affidavit which I have sent but I have had no response or correspondence from the company. This is reflecting bad on my credit report, can someone please call me on *** to resolve this.
